CALDERDALE CARERS PROJECT

CARERS EMERGENCY CARD

Why have an emergency card?
If you look after someone who could not manage without your help, you probably worry about what would happen to them if you had an accident or were taken ill.

The person you care for may be an adult or child, a friend, relative or neighbour.
Calderdale Carers Project runs a confidential 'Emergency Card' system. Any carer can appy for an emergency card.

The Emergency Card system makes sure that the 'looked' after person is taken care of if the carer is taken ill or has an accident.
The Emergency card is a discreet credit-card size and is effective at home or away.

Carrying the Carers Emergency Card will alert the emergency services to the fact that you are a carer and ensure a repsonse.

The Emergency Card:
• Has a 24 hour telephone contact number

• Has a personal number to identify you to the response service
• Contains no personal details, so is safe and confidential to carry
• Is free for carers

IF YOU DECIDE TO HAVE AN EMERGENCY CARD:

Your details
• Details of the person you care for
• Details of two emergency contacts

Will be kept on the Emergency card database administered by NHS direct.

You should ask permission from all the people involved before applying for an emergency card.
If you would like an Emergency card, please write or telephone Calderdale Carers Project at the address and telephone number given.

There is usually someone in the office to answer the phone from 9.30  am -12.30 pm,  Monday to Friday. At other times, you can leave a  message and someone will get back to you as soon as possible  (usually the next working  day)
